In the FollowMyHealth mobile app, you have the option to export your health records. You can either export your entire health record, part of it, or just clinical documents. For Hugo/Kindred, we need the entire health record. To export your records, follow the steps below.
Android App
Step 1: Log into the account.
Step 2: Tap the Menu option and select “My Health”.
Step 3: You can click on any of the sections such as “Conditions”. Click on the “Export” icon at the top right corner of the screen.
Step 4: Here, you can choose what you would like to export. You can export your health records or just clinical documents by either email or fax. Please export your health records by email.
Step 5: Once you’ve chosen to export your health records by email, you’ll enter your email address. Then, you can choose what you would like to be sent (please choose all available options). Once you have chosen what you would like to be sent, click “Send”.
Step 6: You will then receive a warning message about sending your health records by email. Once you get that message, just tap “Ok”. You will then see a black bar at the bottom of your screen letting you know the email has been sent successfully.
Step 7: Once you’ve received the email, save the file to your computer and follow the Upload Tool instructions.
iOS App
Step 1: Log into the account.

Step 3: Tap on the “Export” icon at the top of the screen.
Step 4: Here, you can choose what you would like to export. You can export your health records or just clinical documents by either email or fax. Please export your health records by email.
Step 5: Once you choose your option, you will enter your email address. Then, you can choose what you would like to be sent; please select all options. Then, click “Send”.
Step 6: Once you’ve received the email, save the file to your computer and follow the Upload Tool instructions.
